@Minifrog: Most people today aren't as proper as they were back in the olden days. Lady is kind of old fashioned, and it would make a guy look like he's trying too hard.
When I hear lady I think of a woman in high heels and lots of money. So if a guy came up to me and said lady I would take it as either he's laughing at me (because I don't fit the ladylike description), or he thinks of me as a snobbish, rich person.
