It means the person’s motivation for doing something is to help the mother. Something that is “in a person’s best interest” is something that would be best for them. “At heart” just means that, deep down in the person’s heart, they are trying to do what is best for the mother and not thinking about anything else, for example personal gain.
“It would be in your best interest to take this job.”
“I know you think it is mean of me to break up with my boyfriend, but I really have his best interest at heart. We wouldn’t have been happy together.”
“A: why did the doctor order such an expensive test? He doesn’t care about me, he just wants to make money!
B: He’s been very kind to us. I don’t think he’s motivated by money, I think he truly has your best interest at heart. He must think this test is important.”
