“A” goes before a non-specific noun. But the phrase “it doesn’t make sense” is informally using ‘sense’ as a verb. “Making sense of something”, “to comprehend or understand”. Therefore the article is not needed. “It” would be the subject in this situation. “Make sense” would be the verb.

No. “A” Or “an” if the subject starts with a vowel, goes before a noun. That’s it. It is never used before a verb or after one.
