What does testa di cazzo mean?

It's a quite versatile expression, imho.
Like dickhead, it's an insult you use towards somebody who said / did something very stupid that (in most of the cases) created a mess.

You can use it directly towards the person you're talking to, like:

"Guarda che hai combinato, testa di cazzo!"

Or also when you're talking to a friend and need to pour out your anger because a certain person made you very angry, like:

"Quello lì è proprio una testa di cazzo, l'altra sera mi ha fatto fare una pessima figura davanti a tutti!"

You could also use it to describe somebody in general, like:

A: "Sai, l'altro giorno ero in centro e ho incontrato F.!"
B: "Anch'io. Ci ho scambiato giusto due chiacchiere la settimana scorsa. È sempre la solita testa di cazzo."
