mi sento soffocare quando c'è un esame.
I have changed "il" to "un" because "il" refers to a specific exam, and, at least for me, it sounds a bit unnatural to talk about a general aspect of yourself referring only to a specific exam.
Instead, if you use "un", the sentence refers to a general situation, and the meaning shift to "every time I have to take an exam". In this way, the sentence comes to describe a specific part of your behaviour.
