I would focus first in the meaning of this sentence.
Ought not to = should not
Physicians should not assume that their own views about dying are the same as their patients.
Physicians should not think that their own views will be the same as their patients on the topic of dying and where to die.
As to the grammar:
Physicians ought not assume
This is a very awkward phrase.... I myself wouldn’t use this “ ought not to”
