"It's hurt" would be saying "It is hurt." Which would indicate something or someone is in pain. This term is not used much.
"It's hurt because it was kicked by a child." (This sentence describing something with an unknown gender, could be a bug)
It hurts would be saying that you are in pain.
"It hurts to say goodbye."

You can use "It hurts" when something/someone hurts you. You are the one that feels the pain.
ex: You said mean words to me. It hurts (my feelings)
ex 2: I hit my knee on the table. It hurts (my knee hurts)
And you use "It is hurt" when talking about an animal that is hurt.
ex: That dog is limping. It is hurt.

"It hurts" is the correct way to say that you are in pain.
"Is it hurt?" Can be asked if you are referring to an animal(it) and wanted to know if it's in pain. It is grammatically correct, however it sounds weird to my ears. :) In my opinion, it would be better if you ask "Is the animal hurt?" or "Is the (particular animal, for example, dog) hurt?".
"It is hurt." can be an answer to the previous question if the animal is in pain, and "It isn't hurt" if it's not.
But if you want to ask someone if s/he's in pain, you could say "Are you hurt?" or "Does it hurt?"
