New Yorker is a person who lives in New York City. "keep them waiting" means to do something for too long that would cause the other person to wait.
Being an American who doesn't come from New York City, this saying holds no meaning to me. I guess the assumption is the New Yorkers are tough an inpatient. Really, I would say that they are arrogant and self-serving :) But what do I know?

@Tynion You are correct, people from New York City have a reputation of being very impatient and a bit rude sometimes. People from New York have to walk a lot, they have to deal with crowds everywhere, they have to wait for the subway all the time, they have to wait in long lines just to buy coffee, so they get very impatient. Naturally, New Yorkers do not like to be kept waiting.

ironically, I live in the suburb of a smaller city... and I don't like being made to wait either. That's why it's a comical "New Yorker" thing. As if thier time is more important than anyone else's :)
