Question about English (US)

When you use “family”(noun as subject), does the following verb need “s”(the third-person singular 's' rule) or not in these sentences?

My family go to ~.
My family goes to ~.

Your family like ~.
Your family likes ~.

His family play ~.
His family plays ~.

I still have one more question to ask.
When I say “my family”, is it including me? or do I have to say “My family and I” to includ me?
How about when the subject changes “Your family” and “His family”?

These are confusing, please help me.
