"Apply" sounds strange, but it is probably the correct term to use in medical context.
>"you need to flatten it"
I'm not too sure of the meaning of "let", it could be:
>"lift the gathers on both sides up" (The person takes action to bring the gathers up)
>"let the gathers on both sides come up" (The gathers come up as a result of the previous action)
