To blame - you are the one blaming someone
To be blamed for - You are the one being blamed (by someone else)

'to be to blame for' is to have done something/caused something/being responsible for something, with 'You' as the topic. 'Be blamed for' is should/is supposed to blamed for doing something. With the second being more hypothetical and the first being true.
