La diferencia esta en el uso.
Disculpe se usa regularmente cuando te diriges a desconocidos y se usa para llamar la atencion de alguien o para disculparse como cuando usas I'm sorry.
Ej. Disculpe señor. Dices si tropiezas con alguien.
Disculpe señor, puede decirme que hora es?.. Si preguntas a alguien desconocido a tu lado.
Disculpame: su signigicado es "disculpas por mi" Se utiliza regurlarme con personas que conoces si quieres disculparte por algo. Ej. Disculpame por no llamarte ó Disculpame, ahora no puede hablar.
Tambien puedes usarlo para dirigirte a un desconocido para llamar su atencion,si no quieres usar formalidad.
Ej. Le quieres preguntar a una persona menor que tu la hora y aunque no lo conoces puedes decir: Disculpame, Que hora es?..
Disculpen es para dirigirse a mas de una persona, los conozcas o nó, ya sea para llamar su atencion o para disculparte.
Ej. vas por la calle y hay dos personas y quieres preguntar una direccion. Dices: Disculpen, como llego al Parque Washington?..
Llegas tarde a una reunion con amigos y puedes decir. Disculpen, no pude llegar a tiempo.

Disculpe is a formal way of saying excuse me, and you would most likely say it to a stranger when you're asking them for something
Example:
"Señor!, disculpe, sabe donde está la estación de tren? No la puedo encontrar."
Disculpame is another way of saying excuse me but to close friends, family, acquaintances, etc.
Example:
"Disculpame Lucas, sabes donde está la estación de tren?"
Disculpen is yet another way of saying excuse me, but this one is plural, and can be used in both formal and informal scenarios.
Example :
#1 "Disculpen Lucas y Mónica, saben donde está la estación de tren?"(informal)
#2 "Disculpen, acaso saben donde esta la estación de tren?" (formal, to strangers)
