When I say something in German and I need to use an article, do I use the article of the noun or the adjective?
For example, would "Do you want a baby sister" be, "Willst du eine Baby Schwester?", or "Willst du ein Baby Schwester?"

"Baby" is not an adjective, and in German adjectives follow the gender of the respective noun they belong to, so the question is kind of moot.

"Baby Schwester" is not a possible expression in German. You'd have to use "eine kleine Schwester" and the article refers obviously to Schwester in this case. If you're asking your child or sibling, whether they would like to have a baby sister, that is already implied.
In general the article always refers to the noun. Adjectives do not have articles (luckily for all of us).

@KikuJones If the baby hasn’t been born yet and you don’t know whether it’s going to be a boy or a girl you might also ask: “Willst du ein kleines Geschwisterchen?”, which is a gender neutral term.
