It can mean roughly the same thing.
But "I don't know what to say" could also mean I don't know what topic to discuss or what thoughts to express.
Whereas "I don't know how to say it" generally means: I know what thoughts I want to express, but I don't know the best way to articulate it so that it comes across in the way I want it to.
