There isn’t a lot of difference. Both imply risk, but saying “take a risk” simply suggests a greater chance of failure.
“Take a chance” is a more positive, more hopeful phrase.

They may mean the same things sometimes, but if you say 「take a/the chance」 then that would imply that the "chance" being taken will only happen rarely, so「take the chance」would mean that you saw the chance of something happening and you decided to take the opportunity to do it.
For「take a/the risk」it can mean that if you are faced with a life or death situtation, wherein if you decide to "take the risk" to keep going into a dark forest instead of stop and sleep.
「We took the risk and decided to venture forth in the dark forest.」
「I decided to take the chance to show my new project to you all.」
