“Year old” is singular, “years old” is multiple.

This is a ten-year-old boy.
when it's adjectival you don't make it plural

In these sentences, we only say "year" when the person is one. We say "years" for all other numbers. This is because "year" means one year, and "years" means more than one year. (singular and plural)
The boy is a 1 year old.
She is a 3 year old.
He is an 11 year old.
He is a 30 year old.
When using "a" or "an" we always say year.
