"Did she ask for us by name?" means that the person asking wants to know if "she" has requested for specifically the person asking and whoever is represented by "us". Asking by name is when you request for a person or persons by their name, implying some level of familiarity between the person requesting, and the person requested for. Alternatively, someone could request for someone without using their name such as "I would like that person of there." which does not imply any familiarity. When someone asks for someone else by name, it generally implies that the person requested for has some quality that the person requesting wants, and is in some way perhaps valuable, or contrarily, that the requested person has some negative quality, such as a naughty child being asked for to be told off.
Hope this helps, feel free to ask anything about what I've said.

It's a question someone asks in a situation wherein there might be many people with the same name? and that someone needed a specific person, thus asking for someone by name.

It means "Did she ask for anyone (to help or to do the task) or did she ask for ME to come help?"
For example, if I'm cooking and need help, I might say to my family, "Hey can someone come help, please?" In this case, anyone is okay.
However, if I want my dad to come help me, I would ask my sister "Hey, can you tell Dad to come help me?"
I wanted *his* help specifically, so I 'asked for him by name'.
Hope that helps!
