It means whoever is speaking has a friend that isn’t a she, or a he. They’re a they. So instead of calling that friend “she/her” or “he/him” you would call them “they/them”.
For example!
“My friend is really good at the piano. They have been playing it for three years.”

@Quinn1 In a way, yes. Some people don’t like she/her or he/him pronouns, so they use they/them pronouns. Just as you would call someone “they” when you don’t know their gender, you call that person “they”. You would use “they” if you don’t know the gender of the person you’re talking about, or if someone tells you that their pronouns are “they/them”.
“This is my friend, Raina. I’ve known them since childhood.”
