A loan is an actual giving of value. Once you have taken a loan then you have a debt. Debt is owing something to someone else, as in, "a debt of gratitude."

Debt is an uncountable noun for the total amount of anything owed, either money or something symbolic like gratitude. A loan is a countable noun for a single act of lending money or an object.

Debt means that you are in a position in which you owe money or you have loss of money. An example is "I'm in debt." Here, you're clearly stating the financial situation you are in.
Loans are what you describe borrowing money. It can be either from a company, insurance, person, etc. An example of this would be, "I'm taking out loans." It's the same as saying, "I'm borrowing money."
