@Kazujo So sorry for the late reply! "It's called" is just an expression in this scenario; that's the best word I have for that phrase in this context. "-People having to go through a tough life and sometimes you have to work-" is the "name" of what she's talking about.
1. When someone asks "what's this?" you can say "it's called ___." That's literal.
2. If someone says "you don't have two kitchens in your house??" in a confused way, you can say "it's called not everyone is rich," as a sort of snarky response to their ignorance. That's metaphorical.
I can't rewatch the video at the moment (sorry!), but I believe the employee was questioning the validity of the aunt's condition, so the woman responded to the employee's ignorance with that "it's called ____" construction. It's not a very polite way to explain something to someone (in a situation like ex. 2), but if you're not aiming to be polite or kind and are trying to get your point across strongly, then you can use it.
