Does this sound offensive?

If a person is explaining about something and says to you, “do you understand what I’m saying?”

I’ve watched some YouTube videos and some Americans were saying you shouldn’t say, “do you understand?” because it would sound like “do you have the ability to understand?”

Is this true for all Americans or only for some people?

Do you get offended even if a non-native speaker say that?

Or, is it the matter of how a person say it, like a tone of his or her voice?
