"Understand" and "realize" are similar concepts, generally to do with thinking and knowledge. Depending on context, you may be able to use either, but only for ideas - you can "understand" a person, but you cannot "realize" a person.
Realizing something generally involves putting facts together in a way that give you new knowledge - "I thought about our conversation and realized she didn't like me" or "I looked at the clock and realised I would be late for work".
Understanding generally describes a state of mind - "I understand calculus" or "I understand German". It can also be used to check that you have a correct idea of what another person is saying - "As I understand it, you mean" - or to empathize with them - "I understand what you are going through." Funnily enough, realising something can give you better understanding.

The grammar in that sentence is not entirely correct - it should be "I truly understand" or "I have truly understood". You could also put quotes around "live your life as a movie you would want to see."
