On the head. I just looked it up and “in the head” may be more British. If you were at a hospital and were describing what happened you could say “my friend got slapped / smacked in the head.”

There's no rule I've come across... even if there is one, it probably doesn't apply in most situations. Since elementary school I've been taught a bunch of English rules that never work-- for example, I could name at least 5 words off the top of my head that have e before i and aren't after c

They’re expressions and usage you just learn in childhood as in any other language. In this particular case there’s no rule. Yes, it’s crazy. 😛
