Couldn't = Was not able to
So "I couldn't understand" = "I was not able to understand," you did not have the ability to understand, it was difficult for you and you were not able to do it
"Didn't understand" lacks the nuance, the point of not understanding is still there, but you're not explaining why you didn't understand.
