Most people would interpret them to mean "This is a picture of YOU when you were at your grandmother's house". In most cases no one would be confused by any of these.
"This is a picture of when you WERE at your grandmother's house."
It is a picture of "that time when you were at your grandmother's house", so this could also be a picture of your clothes on the floor at her house, proving you are there, but you aren't in the photo.
(most of the time you would say "were" not "are", unless you are saying that the picture looks like every time you visit, all at once)
"This is a picture of you at your grandmother's house."
This could also mean that the picture of you (the physical object photo in a frame) is sitting in your grandma's house, and the picture could be of you graduating college.
