You can use “for the worse” as in “He took a turn for the worse.” This means he did not get better but in fact his condition deteriorated. You can’t say “to the worse”.
There is an expression “for better or worse” and it means no matter what the outcome is
We’ve made our decision and will continue with it for better or worse.
