they are all obviously falling.. it's just the style and description.
to fall down is in a downwards direction ..
to fall off means you were on something then fell off..
and falling over means there was something in your way to trip or fall over ..
I didn't see the trap door and I fell down the hole
I tried to get on the horse but it moved and I fell off.

They are all similar and could describe the same event depending on perspective.
I was rock climbing and after getting to a difficult part I fell off. I fell down the ravine and after rolling a few times I fell over the ledge into the river.
