they're all largely the same and can be used interchangeably. however, most usage involves exaggeration. I've called a lot of people and actions 'insane' but I've yet to meet a truly insane person.

Also different usage and connotations.
"He is mad about skating" - He just loves skating
"We got crazy at the party" - We had much fun and were very excited
"You drive me crazy" - You make me irritated and angry

yes but in all the examples you list, at least two of the three can be used.
he is crazy about skating
we had an insane party
you drive me insane/mad/crazy
