Answer Are you hungry yet?
Both! To ask if somebody wants to go get something to eat with you, you would ask "Are you hungry yet? Want to go get lunch?"
To ask if somebody is already hungry, even though you both only ate an hour ago, you would ask "Really? Are you hungry already? We just ate!"

It's better(and even more natural) to say : Are you already hungry?
