A: This is grammatically correct, but the phrase "to exact revenge" sounds a little bit old fashioned. These days we tend to just say to get revenge. So the most natural phrase would be:
he got revenge on his ex for cheating (on him).
It would also sound natural to remove "on him", but it's fine to include it as well.
