The difference is that the first one is grammatically correct and the second one doesn't make any sense at all lol
But you could say: "Who has problems?"
In this case, you're asking who is the person that has problems.
When you say: "Who doesn't have problems?"
You're like: everyone has problems, who doesn't? Having problems is normal, so I shouldn't be so pissed off lol
