"Hemma" is used as;
"Jag är _hemma_" = "I am at home"
While "hem" is used as
"Jag ska gå _hem_" = "I am going to go home" OR
"Detta är mitt/ditt/deras/vårt _hem_" = "This is my/yours/theirs/our home"

'Hem' is usually the physical home (the house for example) or when you are not at the home, while 'hemma' is used to refer to the location when something is happening in your house or that is where you are. (if this makes any sense)
Hans hem är väldigt trångt.

Jag var hemma hos en kompis.
