If you have a cake and you eat it, you don't have it anymore--it's gone. You can either have a cake, or you can eat it, but not both.
It is a saying which means that you can't have two opposite situations, no matter how much you wish you could.
For example, if someone likes to be alone but also feels lonely, they can't have their cake and it, too--either they must be alone, or they must be around people so that they don't feel lonely.
(This saying confused me for a long time because to "have" food can also mean to eat it.)
