If you are referring to a soccer game it would be better to say that "Mike was taken out and Nick was put in", because Mike was taken out of the game while Nick was put into the game.

You can use "taken off" (as in taken off the field) but not bring out (that has a completely different meaning). Likewise you can't use "put on" but you can use "put onto" (for put onto the field). As for "bring on" it would be better to use past tense, as in "brought on" (as in the player was brought onto the field.) you could also say that a player was "brought into" the game.

It's more common to say that a player was subbed in than a player was subbed off. Usually if a player is coming off the field one would say that Player Y is going to sub for Player X.

Yes that would be fine. But you don't have to say "he" if you're referring to the coach. Commonly commenters will say something like "Mike was brought on for Nick", since the "he" part is usually already implied.
