to do a language exchange
is a verb
and exchange languages is as if people would exchange money for example
if you want to say you did a language exchange you say "I did a language exchange last year" for example
but if you want to say that two people exchanged their languages you say "He and she exchanged languages"
but i think you mean the first one so the first one should be correct.
