It's mainly the accent and some differences in the vocabulary. There are also some different grammatical structures

The written language (i.e in books, official documents, etc) is exactly the same but the spoken language is wholly different. Expressions, vocabulary, etc differ a lot so it's difficult for most non-Canadian francophones to understand. Europeans and other francophones can understand it easily when it's spoken by someone on TV or when it's a more or less "official" situation, but not when it's spoken casually. Some movies in Canadian French have French subtitles for non-Canadian viewers. Overall it's mostly because most people are not familiar with the expressions and the way of pronouncing words over there, and they also tend to use more English words than other francophones
