walked out on me (to walk out on) = to leave the relationship without saying why (break up unexpectedly)
spell it out = to say something literally or obviously, to make a message more clear
fifth date revelation = she realized something after a while, like after going on 5 dates

Walking out on someone means like leaving someone suddenly, at once, without any heads up or something.
About the noodles :
Monica was simply saying the same thing as "If you're too embarassed to talk about it, would you like to write it down?" Only, instead of suggesting he get a pen and paper, she makes a joke, suggesting he use the noodles on his plate to write what he is too shy to say.
Fifth day relegation :
It's like he surprisingly found out something about his date on their fifth date, and it's kinda funny and became like an expression, it's like he found out something that could happen only on the fifth date, normally sex related stuff.
