You can also say "I have rough skin on my knees". You don't need to use the "a" in the sentence.

To be honest, the "a" That was used in the sentence wouldn't be. Just saying "I have rough skin on my knees" would make more sense. Since your not talking about a skin because you would have a lot of skin. It just sounds more natural that way.
