You are almost right, usualy ばあちゃん is not respectful, however, it sometimes depends on the way one speaks or his/her character.
In some cases ばあちゃん might sound very, very friendly or cozy.
It would be better to carefully see what kind of person uses ばあちゃん in what kind of situation.

I think rude person or children use ばあちゃん。

they are same , some people say お*ばあちゃん、some people say ばあちゃん…
ばあちゃん…is not rude , just casual to call one's grandmother . sounds like friendly (familiar) .
お*ばあちゃん…all purpose any way. polite and decent.
