You should say "The sky".
No I'm joking. You could say:
"Nothing much"
"Hey I'm great, how are you?"

@Pajh: I personally HATE this phrase with a passion. Whenever someone says this so me I always say "the sky" or "your balls if you don't shut up" 😂 but as they have said the general answer is something like "not much"

@Rakhlenko: it's because the sky is up. So if someone asks a stupid question like "what's up?" you give a stupid reply like "the sky (is up)" or "the ceiling" if you are inside 😆

@petrvaganov it sounds like you're making fun of the one who's asking this question. As it's not meant to answer what is up, it's used as a greeting. I normally reply hi or not much as our British friends have just recommended 😀
