Both "ばあちゃん" and "祖母" have same meaning "grandmother".
But "ばあちゃん" is a very casual expression, and "祖母" is a formal expression.

“ばあちゃん” is mainly used when you call your own grandmother. Example: おばあちゃん!久しぶり!(Hi, grandma! It’s been a while!)
Also, sometimes it’s used when someone speak to an old lady. Example: おばあちゃん、荷物重そうですね。持ちましょうか?(Hi, your luggage seems to be so heavy. May I help you to bring it?)
“祖母” is used when you talk about your grandmother with someone else in a formal way. Example: (私の)祖母は5年前に亡くなってしまったんです。(My grandmother passed away five years ago.)
