祖母 means someones granny but おばあさん could mean either someone's granny or an old woman in general
You say 祖母 only about your own grandmother. If you say 「あなたの祖母」(Your 祖母), it sounds strange and also rude to あなた.
When you are talking about your own grandma,
祖母 sounds more formal than おばあさん or おばあちゃん.
おばあちゃん sounds friendly.
If you are over 20 years old, you should say 祖母 about your grandma. If you talk about her referring おばあちゃん, it sounds childish. if you refer to her by saying おばあさん, you sound as if you are talking about a perfect stranger.

祖母 is formal and おばあさん is more casual(it's also a little bit formal though)

since you're foreigner, you can always call your grandmother "おばあちゃん" .
most of the foreigners living in japan call this way.
it's no problem.
and as for old woman(not your grandmother) , you can say おばあさん。
