You would usually say “on my birthday”. “Hang out” is usually said with friends, but it wouldn’t really sound unnatural with family. With family you’d usually say “I’m going to spend time with my family”.

On my birthday.... the rest is fine.
You can hang out with anyone. Cats, dogs, criminals, family, friends.
If you want to use a more formal phrase replace “hang out with” with “spend time with”.
