“At” - You are physically where the hospital is located.
“In” - You are inside the building of the actual hospital.

Agreed. The difference is subtle. If you are IN the hospital you are definitely inside its walls. If you are AT the hospital you could be inside, or outside the hospital but close by (in the car park, at the bus stop) "I just arrived at the hospital, I am now going in".
To complicate matters further, you can also say "She is in hospital". That means she is a patient (in a bed) in the hospital.
