-> The speaker is sure that Tom definitely doesn't know it at all.
ex. "Would Tom know it?" = Tom doesn't know.
Is this interpretation possible?

Would Tom even know?
If this is for a writing document like a short story, I would put “know” in italics, because when speaking you’re going to emphasize “know?” with a questioning tone.

@hhoc2000 yes with the correct context coming before, it’s fine.
A: “Let’s ask Tom if he knows of any good women’s perfumes.”
B: “Would Tom even know [that]?”
