"Had they just paid attention" is a conditional clause using the past perfect tense. IF they had paid better attention (different condition in the past)... THEN the RESULT would have been different (i.e. "Questions could have been answered"). In this sentence, the different possible result (could have been answered) is stated before the antecedent condition is clarified ("had they just paid attention"). Hope this helps.
