"As-tu besoin d'aide ?" is not informal. :)
Inverting the subject/verb and adding a "-" makes the sentence "formal" so even if you use "tu", the phrase is still formal.
What's different is the "politeness" level.
Using "tu" or "vous" might not be appropriate depending on whom you're talking to.
One of the most important thing to understand are as follow:
You shouldn't use phrases such "As-tu ... ?" with friends/family or anyone you call "tu".
Use "Est-ce que".
Same for "Avez-vous ... ?", even if it's formal/polite, it's not common orally and not recommended for learners. It sounds too strong.
You should almost always use "Est-ce que ... ?" which is neutral, common and usual way of asking questions.
To summarize:
Avez-vous besoin d'aide ? formal/polite
As-tu besoin d'aide ? too formal X
Est-ce que vous avez besoin d'aide ? usual/common/recommended
Est-ce que tu as besoin d'aide ? usual/common/recommended
Vous avez besoin d'aide ? informal/be careful to whom you speak to
Tu as besoin d'aide ? informal/familiar/very common with friends/family etc.
