A man is standing on a boat.
"He couldn't catch his balance, and over he went."
This means he went over the side of the boat because he fell.
But now people use it for other situations too, not just being on a boat.
If a man is walking and he trips and falls over you can say "he tripped and over he went".
The meaning is "over he falls" with "went" being past tense. In other words, "He went and fell over."

It depends on the context but it means he fell (down) over (there). If he was balancing on something like a tightrope or riding in a boat, it would mean he fell off the tightrope or fell out of the boat.
