The person was involved in the Battle of Loos. The person was injured during the battle. A small, very sharp piece of metal (shrapnel) hit the person on the back of the neck, “low down” means it is probably closer to the shoulders than to the head.
“I forget if I mentioned” means that the person telling the story isn’t sure whether he/she already told you about the injury, so he/she is going to tell you again.
