In real life, you don't even have to say "I have to manage this" as it's typically mutually understood.
Some other possible examples would be
1. May I have your name for tracking purpose?
2. May I have your name? I need to track all loaner devices.
3. Would you write down your name here?
4. No need to fill out any form. I just need your name for tracking purpose.

@English_learning Are you in IT? Yes, you can start the process by asking the name and any one of those examples would work. Personally, I like to give a piece of paper and a pen, ask "Would you write down your name here?" Sometimes, it's hard to spell names..
